Informed Consent for Health Information

We have entered into separate Business Associate and Qualified Service Organization Agreements with our customers, vendors and subcontractors to comply with the rules on handling of Protected Health Information under the Health Insurance Portability and Accountability Act (“HIPAA”) Standards for Privacy of Individually Identifiable Health Information, 45 C.F.R. Part 160 and Part 164, Subpart E (“Privacy Rule”), the HIPAA Security Standards, 45 C.F.R. Part 160 and Part 164, Subpart C (“Security Rule”), the HIPAA Breach Notification Regulations, 45 C.F.R. The Informed Consent authorizes your health services provider to disclose the health information listed on the Informed Consent to iTether Technologies, Inc and its contracted agents to complete your enrollment and ensure your access to and participation in the iTether Platform but only in the manner set forth in the Informed Consent.

You can review and change your personal information by logging in and visiting your account profile page. If you are a Platform user who was not granted rights to change your personal information by the iTether customer who purchased your use rights from iTether, please contact the customer to request such rights or changes to your personal information.
